New USB Hub - No ports working?

I just installed a brand new Plugable USB 3.0 Hub and I am unable to get any or the ports to work?

TESTS:
(1) is power on… the power supply is plugged in and the green light on the power supply is ON (do not know if power is getting to hub.
(2) unplugged each item from hub and plugged directly into laptop… WORKED
(3) I have unplugged and replugged the (supplied) USB cable and power cable - NO CHANGE

Anyone got any other ideas?
